When you resolve a MAVL case please complete a case closure form using one of the links below as soon as possible.
If we don’t receive this form, we don’t have any record of the volunteer time you’ve spent on the case or the great outcomes you secure for our clients.
*** Closing pro bono cases is so important to us! Did you know that if Pisgah Legal Services doesn’t get case closing information from volunteer attorneys we might lose some of our grant funding? Please take a moment to complete the form for each case you close. Thank you so much!
General Case Closure Form – Use this form to report pro bono services provided to clients referred through the MAVL program on cases other than domestic violence or landlord tenant cases.
Domestic Violence/50B Case Closure Report – Complete this form to report your pro bono hours and the result of the concluded case.
Landlord/Tenant Case Closure Report – Complete this form to report your pro bono hours and the result of the concluded case. We would like to hear how you have helped achieve meaningful results for your client and about your own experiences during the case.
Self Reporting Form – Use this form to report pro bono services provided to low-income clients outside of the MAVL program.
